Issuance of MDCAT Roll Number Slip:
Students will get their MDCAT Roll Number Slips from PMDC. To obtain your slip, take the following actions:
- Visit the official website of PMDC.
- Locate the MDCAT Roll Number Slip 2025 link.
- Enter your registration details (like CNIC or Application Number).
- Download and print the exam day slip.
How to Download MDCAT Roll Number Slip 2025
- Visit www.uhs.edu.pk
- Click on “MDCAT 2025 Admit Card”.
- Enter your full name on the next page.
- Enter the last four digits of your CNIC/B-Form/Passport (without dashes).
- After selecting the desired search option, print the admission card on plain white A4 paper.

Download Admit Card:
All Punjab candidates can download their admit card from www.uhs.edu.pk from September 2025.
To access the card, enter your full name and the last four digits of your ID (CNIC/Passport/B-Form).
Print the card on plain white, A4 size paper.
Provisional release:
The admit card is issued provisionally, till the fulfillment of the eligibility conditions.
Test Day:
Appear at the designated test center on the date and time indicated on your card. No change in time or center will be allowed.
Important Notes for MDCAT Admit Card:
- The admit card will not be sent out by mail.
- Don’t change or add anything to the admission card.
- A copy of the admission card should be kept for future use.
- The admissions procedure will verify eligibility, thus the issuance of an admit card does not ensure it.
